TRANSPORTATION (WAIVER)

These programs cover transportation costs, ensuring independence for individuals with mobility challenges. Services typically include tailored, accessible transportation options. 

 

 

 

 

ADULT COMPANION SERVICES


Adult Companion Services involve providing social and emotional support to adults who require assistance due to aging, illness, or disability. Caregivers offer companionship, engage in conversations, and may assist with activities such as reading, outings, and light household tasks. The aim is to enhance well-being, foster social connections, and reduce feelings of isolation.

 

 

 

INDIVIDUAL COMMUNITY SUPPORT (ICLS)

ICLS are personalized services for individuals, often with disabilities, enabling them to live independently. These supports include help with activities like cooking and managing finances, aiming to enhance self-sufficiency and social integration, thereby improving overall quality of life within their communities.

 

 

CHORE SERVICES


Chores services involve assistance with basic household tasks such as cleaning, laundry, and tidying up. These services are designed to help individuals, especially the elderly or those with disabilities, maintain a clean and organized living space, enabling them to live comfortably and safely in their homes.

 

 

 

COMMUNITY LIVING ASSISTANCE (CLA)


Community Living Assistance (CLA) refers to programs and services designed to support individuals living independently within their communities. Tailored to specific needs, these services help with daily tasks, social integration, and accessing community resources. CLA programs aim to enhance quality of life, promote self-sufficiency, and foster a sense of belonging in local communities.

HOUSING STABILIZATION SERVICES (HSS)

Housing Stabilization Services (HSS) is a service provided by Link Up Home Healthcare, designed to assist elderly individuals, those struggling with disabilities, and those at risk of homelessness in locating and maintaining permanent housing.
