Link Up Home Healthcare offers comprehensive housing programs throughout Minnesota, serving communities in the Twin Cities Metro and Olmsted County areas, including our Housing Stabilization Services. Housing Stabilization Services (HSS) is a recently launched housing program supported by federal Medicaid or Medical Assistance funding, designed to assist elderly individuals, those struggling with disabilities, and those at risk of homelessness in locating and maintaining permanent housing.
HSS encompasses three key services: Housing Consultation, Housing Transition, and Housing Sustaining.

Housing Consultation
A service to create a personalized housing plan for individuals without case management while assisting the individual in making informed decisions and connecting then to non-housing related services that are identified in their personalized plan.

Housing Transition
A service that helps individuals find suitable housing options

Housing Sustaining
A service that helps individuals maintain their housing
Eligibility

To be eligible for any of these services, you must:

Be on Medical Assistance (MA)
​

Be 18 years or older
​

Have a documented disabling condition (or be 65 years or older)

Experience housing instability
Getting Started
To begin, your case manager typically sends a referral for HSS services to hss@linkup-mn.com. Alternatively, we can reach out to your case manager on your behalf. If you don't have a case manager, feel free to call 612-568-5880, and you can collaborate with one of our Housing Consultants. We're eager to provide you with the support you need!
